What set up do you use exactly? i.e cables that came with the PVR to the TV, then YPBPR cables to either your PS3 or XBOX console
Or a different set up, like AV only cables as your post suggests
I'm confused really, as the instructions, and vids I've seen all say use the YPBPR cables to capture footage with, it's what I use to capture my gaming footage for my PS3 and XBOX footage and I always get excellent results, game and audio are always in sync.
I've not heard of anyone using AV cables to capture any footage on the PVR with before, but I can perhaps see how the video-Audio sync alignment might be a tad out.

"Strange video and audio lag with long MP4 videos, YouTube uploads cannot surpass 15 minutes, $199 is a bit pricey."
"The biggest problem I had with the HD PVR Gaming Edition recording in MP4 format dealt with syncing. I recorded close to 20 minutes of Mass Effect 3 gameplay footage on Xbox 360 before I noticed this. For some reason, the video played just fine within ArcSoft Showbiz. However, when played outside ArcSoft Showbiz, my video’s audio and video quickly got out of sync. It also got progressively worse as time went on.
Check out the video below to see what I mean."
If your videos are longer than 15mins maybe thats why you are having the same problems, other than that, I would suggest going into your settings and seeing if anything has set itself back to default.
And Gabe, I'll see if I can come up with something for you fella, on a personal note, I love HD PVR, it is a bit pricey but a great capture device imo.
