I want to know how to add custom video thumbnails. Does youtube have to give you extra permition or is there just something special you do.

You have to be partnered with youtube to have a custom thumbnail.

There used to be ways on getting a thumbnail but yes DrunkGamer is correct, Partnership is now the only way.
